Clinical Transition Liaison

About the Role:

The Clinical Transition Liaison plays a pivotal role in ensuring seamless transitions from hospital to home or alternative site care. This experienced sales professional uses sales techniques to educate patients, their families, and facility staff about our services and products, promoting timely discharge and therapy independence.

Responsibilities:

* Develop and maintain relationships with referral sources to boost sales and patient starts, serving as the key point of contact for education, assistance, and service.
* Collaborate with area service providers daily to sell our services, conduct assessments to ensure patients are viable candidates for alternative site care, and provide hands-on education to patients and caregivers.
* Effectively communicate with agency staff, medical teams, patients, and families throughout the discharge process to implement effective treatment plans, collect and review pre-admission information, and secure necessary sign-offs.
* Partner with Account Managers and Regional Sales Directors to create and execute business growth plans, focusing on expanding service provider partnerships and delivering educational programs to patient/caregiver and referral sources.

Requirements:

This role requires a Licensed Registered Nurse (RN) or licensed Pharmacist with at least 2 years of experience in the healthcare industry, possessing excellent communication skills and basic PC proficiency. Strong relationship-building abilities, knowledge of standard practices, and customer service skills are essential.

Preferred qualifications include hands-on experience in home or alternate site infusion or discharge planning, growing service provider partnerships, and due to state pay transparency laws, the salary range is $73,916.54-$123,186.89.
