Job Summary

We are seeking a highly skilled Pediatric Nurse Care Coordinator to join our team at Children's Health (Dallas).

About the Role

The successful candidate will be responsible for delivering high-quality patient care, collaborating with healthcare professionals, and ensuring seamless coordination of care.

Responsibilities
  • Assess Patient Needs: Conduct thorough assessments of patients' physical, emotional, and social needs to develop comprehensive care plans.
  • Coordinate Care: Collaborate with healthcare teams to ensure timely and effective delivery of care, coordinating services as needed.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare teams to ensure mutual understanding and respect.
  • Educate Patients: Provide education and support to patients and families on self-care, disease management, and healthy lifestyles.
Requirements
  • Clinical Experience: Minimum 2 years of experience in pediatric nursing, preferably in a clinic or hospital setting.
  • Licensure/Certification: Current license to practice professional nursing in the State of Texas, BLS certification through the American Heart Association.
