About the Role

We are seeking a dynamic and talented Referral Management Clinician PRN to join our team at Behavioral Health Connections in Phoenix. As a key member of our assessment and referral department, you will play a crucial role in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.

Responsibilities
  • Review assessment referrals received from various sources to assess patient needs and determine the appropriateness of services.
  • Assist in coordinating patient care and communicating with referring providers to ensure seamless transitions.
  • Maintain accurate and up-to-date patient records, updating databases and ensuring compliance with regulatory requirements.
Requirements
  • Master's degree in Social Work, Psychology, Counseling, or a related field.
  • Current license in the State of Arizona as a Licensed Professional Counselor (LPC), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW).
  • Preferred 1-year work experience in a behavioral health setting, with experience in assessment and referral coordination.
