Job Summary

We are seeking a skilled LPN to join our team as a Referral Management LPN PRN Varied. In this role, you will play a critical part in reviewing assessment referrals and collaborating with RNs to determine the appropriate placement of patients seeking treatment.

Key Responsibilities
  • Review assessment referrals and work closely with RNs to assess patient needs
  • Assist with coordination of patient care
  • Maintain patient records, updating databases, and ensuring accurate documentation of patient information

Qualifications
  • Nursing Degree from Vocational/Technical School, preferred Associates Degree in Nursing
  • Licensed LPN State of Arizona or compact license
  • Current CPR, if required by a customer hospital
  • Preferred 1-year related work experience
