Referral Management Clinician

We are seeking a highly skilled Referral Management Clinician - Full Time to join our team at Health Connections in Phoenix. As a key member of our Behavioral Health team, you will play a critical role in assessing and coordinating the care of patients referred for mental health services.
Key Responsibilities- Review assessment referrals received from various sources in collaboration with RNs and other clinicians to assess patient needs.
- Assist in coordinating patient care and communicating with referring providers.
- Maintain accurate and up-to-date patient records, updating databases and ensuring timely documentation of patient health information.
- Master's degree in Social Work, Psychology, Counseling, or a related field.
- Current license in the State of Arizona as a Licensed Professional Counselor (LPC), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW).
- 1-year work experience in a Behavioral Health setting, preferably in an Assessment and Referral department.
