NYC Health + Hospitals is seeking a highly skilled Advanced Community Healthcare Professional to join our team. The ideal candidate will provide professional nursing care to patients in group and private homes across New York City.

Job Responsibilities:

  • Assess, plan, implement, and evaluate patient plans of care
  • Provide direct skilled nursing care and instruct patients in their place of residence
  • C Coordinate services of other caregivers as needed

Requirements:

  • A valid New York State license and current registration to practice as a Registered Professional Nurse
  • A valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA)
  • Demonstrated commitment to continued professional development
  • Excellent verbal, written, organizational, and analytical skills

The salary for this position is approximately $85,000 - $110,000 per year, depending on experience.
