Job Summary:
We are seeking a compassionate and skilled Staff Nurse Community Care Specialist to join our team. As a key member of our interdisciplinary team, you will provide comprehensive nursing care to patients in their homes, working closely with healthcare professionals to ensure seamless care transitions.
Key Responsibilities:
- Conduct initial evaluations and ongoing assessments of patient needs, developing personalized care plans that address physical, social, and psycho-social requirements.
- Collaborate with healthcare professionals to ensure continuity of care, providing education and guidance to patients/caregivers as needed.
- Document patient progress, maintaining accurate and up-to-date records in accordance with departmental and regulatory standards.
- Participate in quality improvement initiatives, contributing to the development of evidence-based practices and best practices in home care.
Requirements:
- A valid New York State license and current registration to practice as a Registered Professional Nurse.
- Valid and current certification in Basic Life Support (BLS) through the American Heart Association.
- Ability to work effectively in an interdisciplinary team environment, communicating clearly with patients, families, and healthcare professionals.
Estimated Salary: $90,000 - $125,000 per year, depending on experience.
