St. Croix Hospice is seeking a Hospice Aide

As a Hospice Aide, you will play a crucial role as a paraprofessional within our interdisciplinary care team. Under the supervision of a Registered Nurse (RN), your focus will be on delivering high-quality, continuous, and safe patient care.

Key Responsibilities:
  • Provide essential patient care, including bathing, skin and nail care, dressing, and maintaining personal hygiene while ensuring a clean and organized environment.
  • Engage in respectful and confidential communication with patients and their families.
  • Take and document vital signs, and assist patients with self-administered medications.
  • Plan and prepare nutritious meals, including shopping and feeding assistance as required.
  • Monitor and report any changes in the patient's condition to the RN Case Manager.
  • Assist with mobility, exercises, transfers, and positioning to support a full range of motion and therapy activities.

Qualifications:
  • Must be at least 18 years of age.
  • Active Certified Nursing Assistant (CNA) designation, compliant with state-specific requirements.
  • Licensed and insured driver with a reliable vehicle.
