As a Hospice Aide, you will serve as an essential member of our interdisciplinary care team, working under the supervision of a Registered Nurse (RN) to ensure high-quality, continuous, and safe patient care.
Key Responsibilities:
1. Deliver compassionate patient care, including bathing, grooming, dressing, and maintaining personal hygiene while ensuring a clean and safe environment.
2. Foster respectful and confidential communication with patients and their families.
3. Accurately take and document vital signs, assisting with self-administered medications as necessary.
4. Plan and prepare nutritious meals, including shopping and feeding assistance.
5. Monitor and report any changes in the patient's condition to the RN Case Manager.
6. Assist patients with mobility, exercises, transfers, and positioning to support their therapy activities.
