Addiction Medicine Specialist

The Medical Provider in this role works in an acute withdrawal management setting as well as in a residential treatment environment, caring for patients suffering from substance use disorders. This role is part of a larger addiction medicine team, including experienced physicians, advance practice providers, and other healthcare professionals. The provider performs admission physicals, formulating and documenting a patient-centered plan of care for both management of withdrawal and initiation of maintenance medications to manage substance use disorders. Additionally, the provider addresses other medical issues that arise in detox and residential clients as needed, and participates in coordination of care with other team members internally and externally. This role will spend an average of approximately 30-40 hours per week, in a 3-4 day work week, with 24 hour on-call access to provide admission orders for evening intakes and to triage occasional issues as they arise.
Essential Functions
The provider screens referrals for potential admission to addiction medicine services, conducts histories and physicals on patients within 24 hours of inpatient admission and within 72 hours of outpatient visit, and documents assessment and plan in the electronic medical record. The provider reviews nursing assessment done at inpatient admission and issues standing orders, and orders necessary labs and review results with patients. The provider also orders and interprets urine toxicology screens, and accesses the Oregon Prescription Drug Monitoring Database when necessary. Additionally, the provider provides counseling and education on medication options for the treatment of various substance use disorders, and formulates and initiates treatment plan with patient's informed consent and participation. The provider titrates or adjusts medications as needed, and administers medications for SUD treatment via subcutaneous/intramuscular injections, using standard practices including clean technique. The provider uses motivational interviewing and active listening to support clients recovering from drug/alcohol dependence, and addresses other general health concerns as they arise, including contraceptive counseling, STI counseling and treatment, routine psychiatric care, emergent/temporary management of issues such as hypertension and diabetes, and treatment of chronic viral infections. The provider completes and signs medical orders in a timely fashion, reviews outside health records when necessary, and documents diagnosis, level of service, and procedure codes using EMR. The provider also provides after hours support to nursing, counseling, and respite staff, and coordinates care both internally and externally as needed. The provider assures quality of care by adhering to therapeutic standards, measures health outcomes against patient care goals and standards, and follows detox and Medical Board philosophies and standards of care set by the state Medical Board, and OAR's for detox programs. The provider performs other related duties as assigned and approved by the CMO and BestCare Administration.
Organizational Responsibilities
The provider performs work in alignment with the BestCare's mission, vision, values, and supports the organization's commitment to diversity, equity, and inclusion by fostering a culture of open-mindedness, cultural awareness, compassion, and respect for all individuals. The provider strives to meet Program/Department goals and supports the organization's strategic goals, adheres to the organization's Code of Conduct, Business Ethics, Employee Handbook, and all other relevant policies, procedures, and other compliance needs. The provider understands and maintains professionalism and confidentiality per HIPAA, 42 CFR, and Oregon Statutes, and attends required program/staff meetings and completes assigned training timely and satisfactorily. The provider ensures that any required certifications and/or licenses are kept current and renewed timely, and works independently as well as participates as a positive, collaborative team member. The provider performs other organizational duties as needed.
