VITAS Healthcare is seeking a skilled and compassionate Nurse Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families facing terminal illness and bereavement.
Key Responsibilities- Case Management and Supervision: Coordinate patient care, ensure continuous assessment of patient and family needs, and implement the interdisciplinary plan of care.
- Quality Improvement: Participate in the Quality Assessment and Performance Improvement (QAPI) Program and related activities, including Performance Improvement Projects (PIPs) and gathering clinical data for quality measures.
- Professional Development: Develop and achieve professional growth goals and objectives, participate in orientation and in-services, and engage in training activities to enhance skills and knowledge.
- Customer Service: Provide exceptional customer service, maintain focus on the patient and family experience of care, and serve as a patient and family advocate.
- Licensure: Currently licensed to practice nursing in the state where the VITAS program is located.
- Experience: A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
- Transportation: Reliable transportation with adequate insurance coverage for driver and passengers.
The physical demands for this role include:
- Assisting in lifting and transferring patients, as required.
- Lifting objects in excess of 20 pounds.
- Standing or walking in excess of one hour per day.
- Talking and hearing.
Specific vision abilities required include close vision, distance vision, and the ability to adjust focus.
