Key Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop individualized plans of care.
- Collaborate with healthcare teams to ensure continuity of care and effective communication.
- Provide education and support to patients and families on end-of-life care and symptom management.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure comprehensive care.
- Current licensure as a Registered Nurse in the state where the VITAS program is located.
- A minimum of two years of experience in hospice, home health, or community health in the last five years.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
