Position Title: Supportive Housing Services Coordinator

Key Responsibilities:

  • Collaborate as an integral part of the Community Services Division team
  • Engage in outreach efforts and provide support to individuals in Permanent Supportive Housing (PSH)
  • Facilitate services and meals for participants, including intake processes and crisis management
  • Assist the Case Manager with ongoing case management activities

Core Functions:

  • Ensure the safety and security of staff, participants, and volunteers
  • Be well-versed in emergency protocols and respond effectively to urgent situations
  • Conduct screening, assessment, and referral services
  • Communicate and uphold program regulations
  • Support administrative duties and maintain data tracking
  • Foster positive relationships with community stakeholders

Qualifications:

  • High school diploma is required; a Bachelor's degree in a human services discipline is preferred
  • At least 1 year of experience in social work, crisis intervention, or counseling is necessary
  • Ability to engage with diverse populations in a respectful and non-judgmental manner
  • Proficiency in Microsoft Office Suite
  • Excellent verbal and written communication skills
  • Possession of a valid driver's license and access to reliable transportation
