Salary:
The estimated salary for this position is $92,500 - $97,500 per year, depending on location, experience, and education.

Job Description:
We are seeking an experienced Registered Nurse (RN) to join our team as a Hospice Case Manager. As a member of our interdisciplinary team, you will provide comprehensive care to patients with life-limiting illnesses, focusing on symptom management, pain control, and quality of life. Your responsibilities will include:
  • Assessing patient needs and developing individualized care plans
  • Providing direct patient care, including medication administration, wound care, and vital sign monitoring
  • Collaborating with physicians, therapists, and other healthcare professionals to ensure seamless care delivery
  • Educating patients and families on disease management, medication use, and self-care techniques

Required Skills and Qualifications:
To be successful in this role, you will need:
  • A current RN license in good standing
  • 1+ years of experience in hospice, home health, or palliative care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Compassion, empathy, and a passion for delivering high-quality patient care
