Job Summary

This position offers the opportunity to work as a Hospice RN Case Manager, responsible for delivering high-quality care to patients and their families. The successful candidate will have excellent communication and interpersonal skills, with the ability to develop strong relationships with patients and their caregivers.


Key Responsibilities
  • Makes initial nursing evaluations to determine patient eligibility for hospice services within 48 hours of referral.
  • Identifies patients' and families' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
  • Initiates and coordinates patient care plans, maintaining accurate clinical notes regarding patients' conditions.
  • Collaborates with patients, families, physicians, and other healthcare professionals to provide comprehensive care.

Requirements
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Compensation

The estimated salary for this position is $80,000 - $110,000 per annum, depending on experience and qualifications. Benefits include employee-focused wellness programs, empathetic leadership, and a supportive team environment.
