  • Community Nutrition Program Leadership
    • Annually oversees multiple adult health promotion and management initiatives, providing guidance to contracted instructors.
    • Plans, organizes, executes, and assesses community-focused nutrition education and care initiatives for chronic conditions affecting both youth and adults, including obesity, diabetes, cardiovascular diseases, and other long-term health issues.
    • Develops strategies for the enhancement or modification of existing programs and services to adapt to the evolving needs of the community.
    • Collaborates with various organizations to enhance the nutritional well-being of the community, including schools, businesses, and senior centers.
    • Documents and prepares outlines for programs and classes, including objectives, attendance records, evaluation metrics, and analysis of behavioral changes, utilizing collected data to confirm the effectiveness of services.
  • Patient Support through Care-a-Van
    • Provides supplementary individual nutrition counseling for Care-A-Van patients when additional support is necessary.
    • Records nutritional requirements, patient progress, educational interventions, and follow-up in accordance with the Nutrition Care Process standards while ensuring compliance with privacy regulations and organizational policies.
  • Resource Development and Training
    • Evaluates and identifies suitable educational resources on fundamental nutrition topics based on community assessments.
    • May act as a liaison and mentor for students from local colleges and universities.
  • Clinical Oversight of Community Nutrition Initiatives
    • Provides technical and clinical guidance for all community nutrition outreach initiatives and curricula to ensure adherence to scientific and regulatory nutrition standards.
    • Recruits, supervises, and assesses educational and wellness contractors engaged in delivering nutrition programs or specific instructional components. This includes managing the onboarding process for contractors as vendors and providing necessary IT access.
Shared Responsibilities within the CNO Team
  • Collaborates with the CNO team in planning and executing community events.
Employment Qualifications

Education -

Required Minimum Education: Bachelor of Science

Specialty/Major: Nutrition from an accredited institution

Licensing/ Certification -

Licensure/Certification Required: Registered Dietitian in the State of Practice

Licensure/Certification Preferred: Certified Diabetes Educator

Minimum Years and Type of Experience: 2 years of experience in chronic disease nutritional counseling

Other Knowledge, Skills and Abilities Required: Proficiency in current, evidence-based care delivery methods, emphasizing health promotion and risk reduction. Strong interpersonal skills with the ability to collaborate effectively with patients, non-clinical staff, clinical staff, and project teams. Excellent oral and written communication skills.

Exceptional leadership, organizational, research, evaluation, time management, communication, and interpersonal skills.
