About the Role

We are seeking a highly motivated and compassionate Care Navigator to join our team at Behavioral Health Network. As a Care Navigator, you will play a critical role in providing in-person outreach services to individuals in the community who are involved with the Criminal Justice system and have histories of behavioral health and/or substance use disorders.

Key Responsibilities
  • Provide in-person, telephonic, and Telehealth support to individuals, fostering a collaborative approach within our integrated health model.
  • Meet clients in 24-hour facilities, such as jails, hospitals, and substance use treatment centers, and maintain regular contact to facilitate discharge planning and re-engagement with outpatient services.
  • Serve as the primary point of contact for your caseload, ensuring that each individual's needs are met.
  • Collaborate with providers and under the guidance of the Clinical Supervisor, complete comprehensive assessments and individualized support plans, including safety and disenrollment plans.
  • Engage with all relevant parties, such as jail personnel, probation officers, healthcare providers, and natural supports, to ensure a cohesive approach to care, always with the individual's written consent.
  • Leverage your knowledge of community services to assist clients in accessing resources like health insurance, housing, financial assistance, and educational support.
Requirements
  • High school diploma required.
  • Community Health Worker Certification and/or bachelor's degree is preferred.
  • Bilingual, Spanish and English Speaking is Preferred.
  • Knowledge of mental health and substance use disorders, experience with justice involved populations and justice entities, care management services and how to access community resources.
  • 1-2 years of work experience in a related behavioral health, substance use, correctional and/or related human services field is required.
  • Lived experience of involvement in the Criminal Justice system, and recovery from behavioral health and substance use issues is preferred.
  • Valid driver's license required.
  • Vehicle should be Registered and inspected in accordance with 540 CMR 4.00.
