Nurse Coordinator Job Description

We are seeking a skilled Nurse Coordinator to join our team at Insight Global. This role involves providing professional support, planning, and coordination of patient care for selected patient populations across the continuum of care.

The successful candidate will utilize the nursing process without providing direct patient care, focusing on coordination and care for patients requiring assistance and support in accessing and managing their health care needs.

As an integral member of the care team, the Nurse Coordinator will collaborate with other healthcare professionals to ensure continuity of care and coordination of a wide variety of patient care services.

Key responsibilities include:

  • Coordinating patient communication and education
  • Preparing patients and/or caregivers for procedures and tests in collaboration with the healthcare team
  • Ensuring continuity of care and coordination
  • Identifying opportunities to improve patient care services
  • Assisting clinic management to identify and develop operational needs
