Job Description

The MDS Coordinator role at Avamere Family of Companies involves coordinating patient care plans, scheduling conferences, and managing assessments and reviews. This position requires strong organizational and communication skills to ensure effective collaboration with healthcare staff.

Key Responsibilities
  • Reviewing patient care plans to ensure alignment with facility goals and objectives
  • Scheduling conferences to discuss patient progress and identify areas for improvement
  • Managing assessments and reviews to ensure accurate and timely documentation
Requirements
  • A nursing degree (RN or LPN)
  • An active RN or LPN license
  • Knowledge of RAI process, CAAs, and MDS 3.0

$65,000 - $85,000 per year, depending on experience and qualifications
