Care Coordinator for Individuals with Disabilities in New Jersey

We are seeking a dedicated Care Coordinator to join our team at Community Options, Inc. in Hillsborough, NJ.
Job SummaryThe Care Coordinator will be responsible for delivering high-quality care to individuals with disabilities in a community setting. This role involves utilizing the nursing process to assess, plan, implement, and evaluate patient care. The successful candidate will interact with individuals, their significant others, and health team members while maintaining the highest standards of professional nursing.
Responsibilities- Treatment and Healthcare Procedures: Provide treatment and healthcare procedures ordered by a physician and required by professional practice or law.
- Medical Appointments: Ensure individuals attend medical appointments as per their Individual Support Plan (ISP).
- Supervision and Communication: Advise supervisor of all concerns and issues related to nursing services.
- Medical Reporting: Create and distribute medical/health reports detailing general health, healthcare goals, and physician recommendations.
- Individual Support Plans: Develop and update Individual Support Plans (ISP)/Person-Centered Plans (PCP) with an interdisciplinary team.
- Training and Education: Train employees on the medical needs of assigned individuals.
- Health Monitoring: Monitor health and medical conditions of individuals and refer for medical services as needed.
- Medication Administration: Monitor the use of medications through medication administration logs.
- Supply Management: Ensure health service supplies are available and replenished as needed.
- Documentation and Record-Keeping: Ensure program documentation and billable records are completed accurately and timely.
- Community Engagement: Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
- Bachelor's Degree or equivalent experience in nursing.
- Minimum of one year of experience providing nursing services to individuals with disabilities.
- Valid nursing license.
- Valid driver's license with a satisfactory driving record.
- Complete all state and agency-required training.
- Ability to calculate figures related to medication dosages.
- Understanding and commitment to community-based support for persons with disabilities.
- Ability to exercise good judgment and remain calm in crisis situations.
- Ability to establish a comfortable and supportive relationship with individuals receiving supports.
- Proficient with Microsoft Office.
- Excellent verbal and written communication skills.
- Ability to effectively present facts and recommendations.
- Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community.
- Excellent time management skills.
Work in office, residential programs, day programs, and community environments. May be exposed to changing weather conditions when working outside. Hours and weekly schedules may change due to business needs and may include evening and weekend hours. May be required to be on-call in cooperation with other management staff. Occasional overtime may be required. Frequent lifting, stretching, and other physical exertion may be required. May be required to lift or move 50+ pounds. May occasionally assist with wheelchair transfer of non-ambulatory individuals. May be required to transport individuals utilizing your own vehicle or company-provided vehicles. May be exposed to various medical conditions, communicable diseases, blood, body fluids, or tissues.
