Turning Point Community Programs is seeking a dedicated Assessment Coordinator to join our team in Sacramento. Our organization provides integrated mental health services, employment, and housing to support individuals in achieving their goals.

Job Summary

We are committed to innovative and high-quality services that promote recovery, independence, and self-sufficiency. As an Assessment Coordinator, you will be responsible for completing initial and annual assessments, providing psychosocial, rehabilitation, and recovery-oriented services for adults.

Key Responsibilities
  • Conduct initial and annual assessments to confirm eligibility and identify client needs
  • Develop and implement individualized client plans in collaboration with the treatment team
  • Provide on-going consultation and support to the treatment team and other staff
  • Complete all required paperwork and maintain accurate records
  • Drive on program business and participate in off-site trainings
  • Facilitate groups, provide individual and family therapy, and update diagnoses as needed
Requirements
  • Bachelor's degree in psychology, social work, or related field
  • Minimum of four years' experience as a provider of mental health services
  • California driver's license and current vehicle insurance/registration
  • Reliable means of transportation capable of passing vehicle safety inspection
