Hospice Nurse Career Opportunity with Competitive Compensation

As a skilled RN Hospice Care Specialist at Cantex, you will have the opportunity to deliver exceptional care to patients in their place of residence. This role requires coordination of care with the interdisciplinary group, patient, family, and referring agency.
**Job Summary:**
Primary functions include administering nursing care for patients of all ages, coordinating care with healthcare professionals, and assuming responsibility for care coordination.
**Required Skills and Qualifications:**
* Registered Nurse licensed per state requirements
* 2 years of experience with a Hospice agency preferred
* Good communication skills, assessment skills, and strong clinical documentation skills
* Ability to read, write, and speak English language
* Reliable transportation with valid and current auto liability insurance
**Essential Functions:**
* Conduct comprehensive assessments of patient needs and develop individualized care plans
* Implement and document care plans to ensure quality and continuity of care
* Educate patients and families on disease process, self-care techniques, and end-of-life care
* Prioritize patient needs and collaborate with interdisciplinary team members
* Provide training to other staff as needed
