About the Role:

This RN Case Manager position is an exciting opportunity for a Registered Nurse to join our team at Chapters Health. The successful candidate will provide patient-centered care, manage complex cases, and collaborate with the interdisciplinary team to deliver high-quality outcomes.

Key Responsibilities:

  • Conduct thorough nursing assessments, identify patient needs, and develop personalized care plans
  • Coordinate patient services, including home visits, phone calls, and electronic communications
  • Supervise and mentor LPNs, CNAs, and Patient Care Volunteers to ensure quality care delivery

Requirements:

  • Master's degree in Nursing or related field (BSN preferred)
  • Minimum two years of experience in hospice care or a comparable setting
  • Certification in Palliative Care (CHPN) preferred
  • Current licensure in the District of Columbia, State of Maryland, or Commonwealth of Virginia
