Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at Hospice of Darke County. As a Primary Care Center RN, you will provide direct patient care in our Hospice Care Center, working closely with patients and their families to deliver high-quality, patient-centered care.

Responsibilities:

  1. Provide competent nursing care to assigned patients through skilled symptom management, emotional support, and timely response to patient and family needs.
  2. Coordinate IDG planning for assigned patients and participate in the IDG analysis of the patient, resulting in the development, implementation, and revision of the IDG Plan of Care.
  3. Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
  4. Facilitate continuity of care by coordinating transfer/admission of home-care patients to in-patient setting and vice versa.
  5. Explain hospice services and benefits in regards to Medicare, Medicaid, and Private Insurances, and know when to use Palliative Care Interventions and ABNs.
  6. Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
  7. Order appropriate Durable Medical Equipment (DME) for patients.
  8. Perform competent initial nursing assessments.
  9. Work evenings and weekends as needed to meet the needs of patients and families and agency backup schedule.
  10. Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.

Requirements:

  1. Graduation from an approved School of Nursing.
  2. Current RN licensure in the State of Ohio.
  3. Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
  4. Hospice experience preferred.
  5. Shall have a valid driver's license.
  6. Excellent customer service skills.
  7. Detail-oriented with excellent communication, documentation, and organizational skills.
  8. Ability to follow instructions.
  9. Ability to work independently without supervision.
  10. Travel to other sites as needed. Travel may be in inclement weather.
  11. Ability to lift 25 lbs of supplies or equipment.
  12. Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
  13. Ability to assist with patient care activities such as ambulation, transfers, and positioning.
