We are seeking a skilled and compassionate Part Time Care Center RN to join our team at EverHeart Hospice. As a Primary Care Center Nurse, you will provide direct patient care in our Inpatient Care Center, working closely with patients, families, and healthcare teams to deliver exceptional care and support.
Key Responsibilities- Provide competent nursing care to assigned patients through skilled symptom management, emotional support, and timely response to patient and family needs.
- Coordinate IDG planning for assigned patients, participating in the analysis of patient needs and development of the IDG Plan of Care.
- Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
- Facilitate continuity of care by coordinating transfer/admission of home-care patients to in-patient setting and vice versa.
- Explain hospice services and benefits to patients and families, ensuring they understand their options and rights.
- Perform initial nursing assessments, documenting patient information accurately and thoroughly.
- Work evenings and weekends as needed to meet patient and family needs, maintaining a flexible schedule to ensure seamless care delivery.
- Graduation from an approved School of Nursing.
- Current RN licensure in the State of Ohio.
- Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
- Hospice experience preferred.
- Valid driver's license, excellent customer service skills, and ability to work independently without supervision.
