Job Summary

We are seeking a skilled Primary Nurse to join our team at Hospice of Darke County.

Key Responsibilities:

  • Provide direct patient care with a focus on medical-surgical, oncology, and community health nursing.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements:

  • Graduation from an approved School of Nursing.
  • Current RN licensure in the State of Ohio and eligibility to obtain licensure in the State of Indiana, or current RN licensure in the State of Indiana and eligibility to obtain licensure in the State of Ohio.
  • Minimum of two years of experience in medical-surgical, oncology, or community health nursing, with hospice experience preferred.
  • Valid driver's license and ability to travel to other sites as needed.
  • Excellent customer service skills, with the ability to work independently and as part of a team.
