Competitive Compensation
  • Annual salary range: $82,000 - $86,000

Key Responsibilities
  • Conduct patient assessments and collaborate with the care team to formulate and execute individualized care plans.
  • Refer patients to other hospice care specialists as necessary, or document reasons for not doing so.
  • Maintain accurate and timely patient care progress records in accordance with care center policies.
  • Document changes and outcomes related to pain and symptom management as appropriate.
  • Educate patients, families, and caregivers on effective patient care techniques.
  • Maintain regular communication with the clinical manager and care team.
  • Fulfill on-call responsibilities and provide on-call services to patients and families as assigned.
  • Perform additional duties as required.

Qualifications
  • Minimum of one year of nursing experience.
  • Current RN license valid in the state(s) of employment.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
