About Amedisys
Amedisys is a prominent provider of home health, hospice, and high-acuity care, dedicated to assisting patients and families in navigating the complexities of the healthcare system. With a commitment to compassionate, patient-centered care, you will be joining a team of professionals focused on enhancing the lives of those they serve.
Key Responsibilities
- Conduct patient assessments and collaborate with the care team to create and implement individualized care plans.
- Refer patients to other hospice care specialists as necessary or document reasons for not doing so.
- Maintain accurate and timely patient care progress records in accordance with care center policies.
- Document changes and outcomes related to pain and symptom management as appropriate.
- Educate patients, families, and caregivers on effective patient care techniques.
- Maintain regular communication with the clinical manager and care team.
- Fulfill on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform additional duties as required.
- Minimum of one year of nursing experience.
- Current RN license specific to the state(s) of employment.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
