RN Case Manager

We are seeking a highly skilled and compassionate Registered Nurse to join our interdisciplinary team at Humanity Hospice, LLC. As a RN Case Manager, you will play a vital role in providing exceptional patient care and case management services to our patients and their families.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide high-quality patient care, including administering medications, performing wound care, and managing pain and symptoms.
- Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between care settings.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Graduate of an accredited school of nursing with a current RN license in the state of Oklahoma.
- One to two years of recent experience in acute care and home care settings, with a focus on hospice and palliative care.
- Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced environment.
- Ability to lift, position, and transfer patients, as well as perform other physical demands of the job.
- Valid driver's license and reliable transportation.
