About the Role:This is a part-time position working 8am-4:30pm, Monday to Friday. As a Case Manager, you will be responsible for providing exceptional care to patients and their families, ensuring seamless transitions throughout the healthcare continuum.
Key Responsibilities:- Conduct comprehensive assessments of patients' clinical, psychological, and financial needs.
- Develop and implement individualized care plans to achieve optimal clinical outcomes and financial goals.
- Collaborate with healthcare professionals, patients, and families to ensure timely and efficient patient movement throughout the continuum of care.
- Monitor and report on patient progress, identifying opportunities to reduce costs without compromising quality or outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current RN licensure in the state of Oklahoma.
- Experience in case management, preferably in a hospital setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Proficiency in electronic medical records and other relevant software systems.
- Case Management Certification (CCM) or equivalent.
- Experience with managed care and payer/provider requirements.
- Knowledge of CMS regulations, Medicare/Medicaid, and other payer regulations and benefit limits.
