SSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and are discharged safely from the hospital.
Responsibilities- Assess patients' discharge planning needs and develop plans to meet those needs.
- Collaborate with the interdisciplinary team to identify and address barriers to discharge.
- Ensure compliance with Condition of Participation (CoP) discharge planning.
- Identify and escalate gaps/barriers related to inpatient and observation length of stay.
- Participate in opportunities for improvement in standard work.
- May also be responsible for utilization management process, ED case management process, admission process, and acting as a case manager liaison with post-acute providers.
- Graduate of accredited school of nursing or education equivalency for licensing.
- Three years' acute care experience.
The RN Case Manager will work in a fast-paced environment, collaborating with a multidisciplinary team to ensure patients receive the best possible care. The ideal candidate will be a team player with excellent communication and problem-solving skills.
