Job Title: RN Case Manager

Job Summary:

We are seeking a highly skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating patient care, managing clinical and financial outcomes, and ensuring that patients receive the best possible care.

Responsibilities:
  • Conduct comprehensive assessments of patients' clinical, psychological, and financial needs
  • Develop and implement individualized care plans to meet patients' needs
  • Coordinate with healthcare providers, insurance companies, and other stakeholders to ensure seamless care transitions
  • Monitor patients' progress and adjust care plans as needed
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Provide education and support to patients and their families
  • Document patient information and outcomes in a timely and accurate manner
Requirements:
  • Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
  • 5 years of experience in a variety of clinical settings (e.g. home health, inpatient, physician office, clinic)
  • Experience with managed care and payer/provider requirements preferred
  • Excellent interpersonal, communication, and collaboration skills
  • Computer experience required
