Hospice Clinical Liaison

Alive Hospice is seeking a skilled Hospice Clinical Liaison to join our team. As a key member of our healthcare team, you will be responsible for hospital-based outreach, referral management, and growth in admission volume.
Key Responsibilities:- Collaborate with clinical leadership and teams to identify and nurture potential referral sources and health-related groups for education and outreach.
- Develop and maintain professional relationships with established providers to promote utilization of hospice services and understanding of the hospice benefit.
- Exhibit superior customer service and critical thinking skills at all times.
- Collaborate with medical community and/or patients and families to facilitate and promote eligible hospice admissions.
- Manage patient discharge from the hospital and the admission process from the hospital to home, IPU, or long-term care facilities.
- Coordinate tracking of referral sources, assessment of utilization of Alive Hospice services, and preparation of relationship activity.
- Manage relationships with contracted skilled nursing facilities and long-term care facilities.
- Self-direct work with little supervision.
- Coordinate educational presentations and in-services.
- Maintain referral source spending log.
- Document daily within the EMR and CRM.
- Conduct on-site data gathering within scope of practice to identify needs and provide support to prospective patients, families, and referral sources.
- Communicate with physicians, medical directors, and other healthcare providers regarding patient evaluation and response to plan of care.
- Complete required admission documentation within scope of practice.
- Provide and/or facilitate appropriate interventions during complex patient referral and admission process.
- Registered Nurse with five years of healthcare experience, preferably in hospice sales and marketing.
- Current Tennessee or compact RN license.
- Current and valid Tennessee driver's license and automobile insurance as required by Tennessee State Law.
