We are seeking a skilled RN Case Manager to join our team at Children's Healthcare of Atlanta. As a key member of our care coordination team, you will play a vital role in ensuring the seamless transition of patients from our facilities to their homes and communities.
Key Responsibilities- Develop and implement comprehensive transition care plans for patients and families
- Collaborate with interdisciplinary teams to identify and address patient needs
- Communicate effectively with patients, families, and healthcare providers to ensure continuity of care
- Facilitate care conferences, rounds, and other meetings to drive care coordination
- Monitor and address potential barriers to care, including risk management issues and quality concerns
- 3 years of RN experience with a Bachelor of Science in Nursing (BSN) or a current Children's nurse with 5 years RN experience and an associate's degree in nursing (required to obtain BSN within 2.5 years of hire date)
- 3 years of broad clinical experience, predominantly in pediatric care
- Preferred qualifications include a Master's degree, EPIC experience, and experience in care coordination, case management, discharge planning, and utilization review
