RN Case Manager

Kaiser Permanente is seeking a skilled RN Case Manager to join our team. As a key member of our healthcare team, you will work collaboratively with physicians to optimize quality and efficiency of care for hospitalized members. Your primary responsibility will be to carry out daily utilization and quality review, monitoring for inefficiencies and opportunities to improve care. You will develop a safe discharge plan, recommending alternative levels and sites of care when appropriate. This role requires strong communication and interpersonal skills, with the ability to work effectively with multidisciplinary healthcare teams and patients/families. Key Responsibilities:
- Plan, develop, assess, and evaluate care provided to members, collaborating with physicians and other healthcare professionals to ensure continuity, quality, and appropriate resource use.
- Review, monitor, evaluate, and coordinate patients' hospital stays to ensure timely and efficient delivery of essential services.
- Communicate effectively with hospitalist partners, patients, families, and other stakeholders to develop and implement discharge plans that meet each patient's identified needs.
- Assess high-risk patients in need of post-hospital care planning and develop comprehensive discharge plans to ensure seamless transitions across the care continuum.
- Document all admissions and discharges in the patient's electronic medical record and make post-discharge follow-up calls to patients who are not referred to ambulatory case/care management programs.
Requirements:
- Minimum two years of experience in patient care delivery or completion of a Master's degree in Case Management in lieu of minimum years of experience.
- Associates Degree in Nursing.
- Registered Professional Nurse License (Georgia) or Licensed Registered Nurse (Temporary - Georgia).
- Demonstrated advanced communication and interpersonal skills with all levels of internal and external customers.
- Ability to collaborate effectively with multidisciplinary healthcare teams.
- Excellent time management skills with the ability to work successfully in a fast-paced environment.
Preferred Qualifications:
- Minimum five years of clinical nursing experience in a hospital setting.
- Minimum five years of professional practice experience in an acute care setting.
- Minimum two years of experience in utilization review, case management, and discharge planning.
- Bachelor's Degree in Nursing, Health Care, or Master's degree in Case Management.
