Reports to: Clinical Program Manager
Job SummaryThe Clinical Care Manager II is a key member of our multidisciplinary team, responsible for coordinating client care and providing therapeutic interventions to individuals with serious mental health challenges. This role requires strong assessment skills, a customer service perspective, and the ability to establish trusting relationships with clients and their families.
Key Responsibilities- Evaluate client strengths and needs, identifying treatment goals collaboratively with clients and team members.
- Provide evidence-based treatment interventions, including Cognitive Behavioral Therapy (CBT), Family Psycho-Education, and Case Management.
- Assist clients in accessing needed services, such as medical, benefits, employment, and housing.
- Collaborate closely with community providers, making referrals to other community and mental health agencies.
- Maintain accurate and timely clinical records, adhering to quality assurance standards and County Behavioral Health documentation requirements.
- Work effectively as part of a team, attending meetings and contributing to case conferences.
- Engage in outreach events, providing psycho-educational presentations and information about early intervention for psychosis.
- Master's Degree in Social Sciences, Psychology, Counseling Psychology, or related field, with current California State Licensure.
- Three years of experience providing mental health services to youth or young adults with serious mental health challenges.
- Personal or family experience of overcoming mental health challenges is highly desirable.
- Demonstrated proficiency in Microsoft Office and ability to type 45 WPM.
- Valid California Drivers' License with a clean driving record.
- Knowledge of County Behavioral Health, community resources, and TAY population is preferred.
- Experience with CBT for Psychosis (CBTp) is a plus.
- Strong assessment skills, with reference to DSM-5, and well-developed interviewing skills.
- Ability to establish trusting relationships with clients and their families.
- Openness to learning and implementing evidenced-based practices to fidelity required.
The position requires sitting at a desk approximately 50% of the time, with standing 20% and walking or driving 30% of the time. Ability to perform routine bending, stooping, twisting, and reaching. Lifting may be required occasionally of no more than 20 lbs at a time. The work is performed in an office environment and/or at various community locations, requiring local travel.
