Job Summary:

We are seeking a highly skilled and compassionate Care Manager II, Registered Nurse to join our team at Sutter Health. As a key member of our care management team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Responsibilities:

  • Conduct thorough patient assessments to identify care needs and develop personalized care plans.
  • Collaborate with healthcare teams, patients, and families to ensure timely and effective care transitions.
  • Utilize clinical expertise to review medical records, identify areas for improvement, and implement evidence-based practices.
  • Communicate effectively with patients, families, and healthcare providers to ensure clear understanding of care plans and transitions.
  • Identify and address social determinants of health to promote patient well-being and reduce healthcare disparities.
  • Participate in quality improvement initiatives to enhance patient outcomes and care coordination processes.

Requirements:

  • Current RN licensure in the state of California.
  • Minimum 2 years of experience in acute care or care management.
  • Strong clinical knowledge and expertise in care coordination and transition management.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
