Job Summary

Covenant Health (Tennessee) is seeking a skilled Outreach Specialist to join our team. As a key member of our Clinical Access department, you will play a vital role in providing care monitoring and in-person support services to assigned consumers.

Responsibilities:

  • Meet with the interdisciplinary team to review treatment needs of consumers.
  • Contribute to the development of comprehensive care plans for designated service participants.
  • Work with consumers and their families to understand, acquire, and maintain needed healthcare services and develop a healthy lifestyle.
  • Assess life domain needs of consumers, including living arrangements, financial, vocational/educational, health, social support, leisure/recreational, and emotional/mental health.
  • Assist consumers in developing skills related to independent living and navigating the service delivery system.
  • Provide ongoing linkage, monitoring, planning, and evaluation, and advocacy for team consumers.
  • Participate in inpatient treatment planning to ensure continuity of care.
  • Assist consumers in obtaining medical, dental, and other personal care services.
  • Attend required staff meetings, training sessions, treatment teams, and other meetings as directed.

Requirements:

  • Bachelor's Degree in a health-related field of counseling, psychology, or social work, or must be licensed as a nurse (LPN or RN).
  • Minimum of one year relevant experience if bachelor's degree is not in counseling, psychology, or social work or if not licensed as a registered nurse.
  • Valid Tennessee driver's license Class D and state-mandated minimum insurance coverage.
