Support Worker

At Guardian Angel Senior Services, we are committed to providing high-quality care and support to our clients. As a Support Worker - Community Habilitation, you will play a vital role in helping individuals with disabilities to live independently and participate fully in their communities.
Key Responsibilities:
- To empower participants to make informed decisions about their independence and well-being.
- To assist participants with daily tasks, such as booking medical appointments and managing finances.
- To provide transportation and support to participants as needed.
- To educate participants on basic budgeting skills and financial management.
- To assist participants in building relationships with their community and accessing local resources.
- To work with participants to set and achieve goals, and to provide ongoing support and encouragement.
- To maintain accurate and timely documentation of participant progress and activities.
Qualifications:
- High school diploma or equivalent required; human services experience preferred.
- Experience working with individuals with disabilities or in a home care setting preferred.
- Knowledge of person-centered care principles and practices a plus.
- Ability to research and connect participants with community resources and services.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work varied hours, including evenings and weekends as needed.
