About You
The ideal Outreach Specialist will possess the following qualifications:

Education:
A Bachelor’s degree in healthcare, public health, social work, or a related discipline (or equivalent professional experience).

Experience:
In-depth knowledge of New York State's Medicaid program and Medicaid Advantage Plan. Previous experience in community outreach, healthcare enrollment, or public assistance programs is highly desirable.

Entrepreneurial Spirit:
CINQCARE is committed to addressing longstanding gaps in care delivery for Black and Brown populations. This role is crucial in ensuring CINQCARE effectively fulfills its mission.

Communication Skills:
Exceptional verbal and written communication abilities; capable of clearly articulating and presenting concepts and models to CINQCARE's team, investors, partners, and other stakeholders. Proficiency in Microsoft Office applications and data management is required.

Relationship Management:
Proven ability to cultivate and maintain effective relationships with business leaders and external stakeholders. Capable of working independently as well as collaboratively within a team.

Requirements:

  • Certification as a Certified Application Counselor (CAC) or Medicare Certification.

About the Role
The Outreach Specialist will report to the Director of Care Management, responsible for providing strategic insight, sound judgment, organization, and evidence-based analysis to influence decisions and meet CINQCARE's objectives.

This role is pivotal in promoting and facilitating enrollment in New York State's Medicaid Advantage Plan (MAP) program.

Key Responsibilities:

  • Design and implement outreach strategies to engage eligible populations statewide.
  • Participate in community events, health fairs, and meetings to disseminate information and address inquiries regarding the MAP program.
  • Collaborate with local community organizations, clinics, and healthcare providers to enhance outreach initiatives.
  • Educate potential beneficiaries about the benefits, eligibility criteria, and coverage options available under MAP.
  • Provide clear and accurate guidance on the application process for MAP and assist individuals in navigating enrollment.
  • Create and distribute educational materials, including brochures, flyers, and presentations.
  • Support individuals in completing MAP applications, ensuring all required documentation is gathered.
  • Assist applicants in understanding their rights and responsibilities within the program.
  • Provide support in resolving any challenges encountered during the enrollment process.
  • Maintain precise records of outreach activities, including the number of individuals reached and assisted.
  • Compile and submit regular reports on outreach efforts and enrollment statistics to the relevant department.
  • Work collaboratively with internal and external stakeholders to ensure a coordinated approach to outreach and enrollment efforts.
  • Stay informed about changes in Medicaid policies and program updates, attending relevant training sessions as necessary.

Work Environment:
This position entails both fieldwork and office-based responsibilities. Frequent travel for home visits and visits to healthcare facilities, community partners, and other locations is required, regardless of weather conditions. In-office work is conducted in a standard office environment with conditioned air and artificial lighting.

The role necessitates the ability to travel frequently by car and/or public transportation, communicate effectively with customers, vendors, management, and colleagues, sometimes in challenging situations. Regular use of telecommunication and email for communication is essential. Prolonged periods of sitting may be necessary, and the ability to receive and process information and prepare or inspect documents is required. Occasional lifting of up to 30 lbs. may be necessary, along with good manual dexterity for using common office equipment.
