About the Role
As a registered nurse (RN) on our team, you'll play a vital role in delivering high-quality patient care and making a meaningful impact in the community. You'll work closely with patients, staff, physicians, and other departments to coordinate patient care services and create a supportive environment that encourages professional growth.
Key Responsibilities
- Assess patient care quality and coordinate services with patients, staff, physicians, and other departments.
- Create an environment that fosters professional growth and integrates evidence-based practices into operations and clinical protocols.
- Collaborate with physicians, nurses, and other staff to facilitate multidisciplinary decision-making that aligns with patient goals and objectives.
- Coordinate care and services within the case-managed population.
- Educate physicians and staff on Case Management standards and assist them in meeting regulatory requirements.

Requirements
- Current state RN license.
- Bachelor's degree from an accredited nursing school.
- Minimum 1-3 years case management experience preferred.
