Key Responsibilities- Assess patient needs and develop individualized care plans to ensure optimal outcomes.
- Collaborate with healthcare teams to coordinate patient care services and ensure seamless transitions.
- Utilize clinical expertise to identify opportunities for cost savings and quality improvement.
- Provide education and support to patients and families to promote health and wellness.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
- Current state RN license.
- Bachelor's degree from an accredited nursing school.
- Basic Life Support certification within 30 days of hire.
- ACLS and PALS certification within six months of hire.
- One year of experience in case management or a related field.
