You will assess members' medication history and verify orders for accuracy, promote medication organization & management, and educate members on the importance of medication schedules and proper medication usage.

Key Responsibilities:

  • Develop and implement comprehensive care plans to meet the unique needs of each client
  • Assess and manage medication regimens to ensure safety and adherence
  • Collaborate with caregivers, healthcare providers, and support services to ensure seamless care coordination

Requirements:
