This role focuses on promoting health, wellness, and safety through comprehensive care planning and monitoring. You will assess members' medication history and verify orders for accuracy, promote medication organization & management, and educate members on the importance of medication schedules and proper medication usage.
Key Responsibilities:
- Develop and implement comprehensive care plans to meet the unique needs of each client
- Assess and manage medication regimens to ensure safety and adherence
- Collaborate with caregivers, healthcare providers, and support services to ensure seamless care coordination
- Provide education and support to clients and caregivers on medication management and overall care
- Participate in quality monitoring initiatives to ensure high-quality care
Requirements:
- Current licensure as a Registered Nurse in the Commonwealth of Virginia or compact state of residence
- Experience in geriatric care and medication management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
