Patient Navigator

We are seeking a highly skilled and compassionate Patient Navigator to join our Oncology team at DHR Health. As a Patient Navigator, you will play a vital role in supporting patients and their families throughout their cancer journey.
Key Responsibilities- Provide personalized support and guidance to patients and their families, addressing their unique needs and concerns.
- Assist patients in navigating the healthcare system, connecting them with relevant resources and services.
- Conduct thorough assessments to identify patients' psycho-social, housing, and resource needs.
- Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
- Develop and implement personalized care plans, addressing patients' physical, emotional, and social needs.
- Provide education and advocacy to patients and their families, empowering them to make informed decisions about their care.
- Document all interactions and interventions in our electronic health record system.
- Graduate of an accredited vocational nursing program.
- Valid LVN license in the state of Texas.
- Valid Texas Driver's License.
- Current BLS certification.
- Experience working in an acute healthcare setting, preferably in oncology.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team environment.
- Fluent in Spanish, with excellent written and verbal communication skills.
