Job Title: Community Health Worker

At DHR Health, we are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in addressing the social determinants of health and promoting overall wellness for our patients.

Job Summary:

The Community Health Worker will provide one-on-one non-clinical interventions to support patients in overcoming barriers to care and achieving a positive health outcome. This includes providing education on social services and community resources, collaborating with our interdisciplinary team, and actively participating in community outreach.

Responsibilities:
  • Provide non-clinical interventions to address patients' social determinants of health
  • Offer education on social services and community resources
  • Collaborate with our interdisciplinary team to address social needs and barriers to health and wellness
  • Participate in community outreach and provide referrals based on individual needs
  • Gather data on patients' Health-Related Social Needs and track referrals successfully made to community resources
  • Maintain accurate and timely documentation of interactions, assessments, and referrals
  • Provide weekly follow-ups with patients for 30 days post-discharge to monitor progress and adherence to care plans
Requirements:
  • GED required
  • Bachelor's degree in psychology, public health, or a related field (preferred)
  • Community Health Worker (Texas) Certification required
  • Bilingual Spanish and English required
  • 1 year of relevant healthcare experience required
  • Knowledge of local community resources and healthcare services strongly preferred
