POSITION SUMMARY:

Primary responsibilities include coordinating release of medical records in response to external audit request letters.

POSITION EDUCATION/ QUALIFICATIONS:
• High school graduate /GED certificate or equivalent is required.
• Requires excellent customer service skills
• Graduate of Health Information Management program (RHIT/RHIA) preferred
• Computer skills with knowledge of Microsoft Office suite preferred
• Good written and verbal communication skills required
• Texas Driver's License under clear status with the DMV required.

JOB KNOWLEDGE/EXPERIENCE:
• Hospital Information Management experience preferred
• Medical Records experience preferred
• Working knowledge of computers systems preferred
• Thorough knowledge of principles and practices of Health Information Management preferred
• Knowledge in the methods involved in collecting and analyzing medical record data preferred
• Knowledge in medical terminology and medicolegal ethics preferred
• Knowledge of pertinent Federal, State and local regulations concerning medical records OSHPD data, and reports vital statistics and diseases preferred
• Knowledge of the uses and limitations of medical records preferred

Responsibilities:

POSITION RESPONSIBILITES:
• Coordinates all external payer audits to make sure all proper documentation is obtained by auditing each account and communicate as necessary with appropriate departments and providers in order to meet the deadline
• Maintains all correspondence by scanning into Cerner, enters notes in Paragon, enters Paragon- Release of Information when received and update once mailed out. Including encrypting CD's and contacting the provider with the password
• Able to navigate through Paragon notes before releasing any records to ensure that it is not a merged or combined account
• Compiles a Reconciliation Report by communicating with Business Office on a weekly basis in order reconcile all accounts received
• Compiles audit request log of requests received including the requesters information
• Contacts physician's office to ensure completion of any pending documentation prior to mailing out the requests
• Assist Director/Manager with any special projects
• Must comply with all Policies and Procedures including confidentiality of Patient Health Information
• Performs other duties as assigned.

• B. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.

C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
