RN Field Case Manager

Job Summary:
Nuclear Care Partners is seeking a detail-oriented and deadline-driven RN to join our team as a Field Case Manager. As a Field Case Manager, you will be responsible for providing 1:1 patient care and working closely with non-clinical and clinical team members. The ideal candidate will have a positive attitude, be self-motivated, and possess excellent leadership skills.
Responsibilities:
- Patient Care
- Build relationships with patients and families
- Scheduling, prepare for and attend physician renewal/new patient appointments
- Initial Nurse Assessments and start of care visits
- Medication Interaction checks
- Maintain patient binder in home
- Ensures implementation of the company policies and procedures
- Ensures orientation, evaluation, and continuing education for all staff
- Responsible for the overall supervision of all the staff in the area
Qualifications:
- Bachelor of Science in Nursing preferred
- Current RN licensure in the state of ID
- A qualified candidate must possess a strong sense of professionalism, effective interpersonal skills, and be a true team player
- Previous case management or leadership role(s) which would include Interviewing, hiring, on-boarding procedures, mentoring, and leading are preferred
- Must have a valid driver license and be in good standing
- Must pass a pre-employment screening
