Main Duties:

  • Lead care teams and coordinate care delivery
  • Provide high-quality primary care services to patients
  • Participate in quality improvement initiatives and program development
  • Communicate effectively with patients, families, and healthcare teams

Requirements:

  • Certification, NYS licensure
  • 3-5 years of similar experience preferred
  • Leadership skills and experience managing healthcare teams
  • Proficient with EMR software and Microsoft Office

Estimated Salary: $140,000 - $170,000 per year, depending on experience.
