Job Summary:The TRC-Licensed Mental Health Clinician will be responsible for providing individualized treatment, support, advocacy, and linkage to individuals enrolled in our Personalized Recovery Oriented Services (PROS) Programs, Home and Community Based Services (HCBS), and Community Oriented Recovery and Empowerment (CORE) programs. This role will also involve education and advocacy to individuals and family members as directed by individualized treatment.
Key Responsibilities:- Perform initial intake and assessment/evaluation through individual interview, interview of significant others, and review of previous records.
- Develop a Treatment Plan to address specific identified needs.
- Provide social work and other specialty interventions through individual, joint, or group counseling.
- Document each individual session and write progress notes based on each individual contact.
- Perform case management services for individuals who require case management intervention.
- Perform periodic evaluations to establish patient needs and determine how identified needs can be met.
- Act as liaison with other agencies and departments mutually involved in the individual's care.
- Complete discharge paperwork within regulatory requirements.
- Serve as a coordinator of services for a caseload identified by the Behavioral Health Services Program Manager.
- Provide specific assessments within scope and expertise to individuals enrolled in behavioral health services.
- Meet all quality and productivity standards as defined by management and regulatory/funding entities.
- Comprehensive knowledge of mental health and the Psychiatric Rehabilitation Model.
- Ability to communicate effectively, both verbally and in writing.
- Self-directed and self-motivated.
- Strong understanding of the person-centered approach and knowledge of or ability to learn the philosophy of empowerment.
- Well-organized, able to manage time effectively, and maintain accurate records.
- Excellent written and verbal communication skills, including the ability to effectively communicate with individuals, families, clinicians, and others involved in the provision of mental health services and community supports.
- Professional clinical staff with a master's degree in a mental health field from a program approved by the New York State Education Department.
- Registration or certification by the American Art Therapy Association, American Dance Therapy Association, National Association of Music Therapy, or American Association for Music Therapy.
- Current licensure as a mental health counselor, marriage and family therapist, licensed master social worker, licensed psychoanalyst, licensed creative arts therapist, or licensed clinical social worker.
