Habilitation Specialist Role

As a Habilitation Specialist, you will play a vital role in partnering with individuals receiving supports to develop a comprehensive, integrated, and clear strategy for TRC habilitation services. You will ensure cohesive implementation and focused progression towards each person achieving their life goals and realizing their citizenship with urgency, intention, and determination. You will co-coordinate care for each person's individualized plan with Care Coordination, ensuring seamless integration and effective support.

Key Responsibilities

1. Develop and implement comprehensive, individualized plans that align with each person's life goals and valued outcomes, ensuring accountability amongst habilitation service providers. 2. Coordinate with each person, their Care Coordinator, and the team to develop and implement individualized plans, demonstrating the collective actions and expectations of team members. 3. Ensure regular contact with service providers, providing clear and timely communication of information, and attending team meetings to align assigned habilitation services towards fulfillment of life goals. 4. Develop and monitor milestones that directly correlate to each person's life goals and valued outcomes, overseeing progress and modifying as necessary to effectively move the person forward. 5. Update and maintain comprehensive plans of protection and/or safeguards with each person on assigned caseload and their support team. 6. Develop, monitor, and revise comprehensive, customized habilitation plans by strategically and intentionally aligning the actions and expectations of TRC habilitation services. 7. Assume primary Case Manager responsibilities and adhere to regulatory requirements, ensuring timely and effective support to individuals receiving services.

Qualifications and Requirements

A Bachelor's degree in Human Services or a related field and a minimum of one (1) year of experience in a program serving individuals with disabilities or a coordinated-care environment are required. Experience in coordination of services and habilitation plan design is strongly preferred. A valid driver's license, use of a personal vehicle, and willingness to travel for work or equivalent accommodations are also required.

Estimated Salary Range: $55,000 - $65,000 per year
